New Immunization Requirements for 2007-2008 school year
Children
registering for the first time in the Columbus Municipal School
District will need:
- Age
requirements - entering kindergarten students must be five (5)
years old on or before September 1.
- Two
(2) proofs of residency (utility receipts, lease, homestead exemption,
driver's license, car registration, etc.)

Copy of latest report card.

Immunization slip for Mississippi schools (blue slip).

Name, address, and phone number of previous school attended.

Copy of birth certificate and social security card (passports
may substitute for social security card).
