| 2007-2008 Choral Handbook | Contains all the information pertaining to the LMS Choral Department. |
| Handbook Confirmation Form | Form that should be signed by both student and parent that acknowledges compliance with Choral Handbook and rules. |
| Medical Release Form | This form must be on file for each student in order for them to participate in field trips or competition. |
| Parental Involvement Form | This form is for parents to fill out and volunteer to help with the various activities of the LMS Choral Department. |
| AD FORM | This is the form that any one who purchases and ad needs to complete and turn in. |
| AD INFORMATION | This provides information for the parents about the AD Book Fund-Raiser. |
